Resumen

El prurito es un síntoma cardinal en Dermatología Clínica, siendo manifestación de enfermedad primaria cutánea, así como, en el diagnóstico y seguimiento de enfermedades de compromiso sistémico. El prurito prolongado, intenso y/o recurrente, puede causar cambios dermatológicos secundarios, tales como la abrasión, la excoriación, el prurigo nodular de Hyde (PNH), la liquenificación y lesiones cicatriciales. Es por esto, que el abordaje del paciente con prurito, debe hacerse de una forma integral, ya que, hasta en un 50% de los casos, puede ser el único síntoma de una enfermedad sistémica y potencialmente seria. La Cirrosis Biliar Primaria (CBP) es una enfermedad gastrointestinal autoinmune, de baja incidencia y prevalencia, que tiene como manifestación inicial el prurito hasta en el 60-80% de los casos, e incluso puede ser uno de los síntomas más precoces. Por esta razón, el dermatólogo debe reconocer sus manifestaciones clínicas, para una aproximación diagnóstica y terapéutica. Se presenta el caso de una paciente de 58 años, con cuadro de prurito crónico y PNH secundario a este, quien había previamente recibido varios tratamientos (incluyendo ciclosporina), sin presentar mejoría, en quien se estudió y se confirmó el diagnóstico de CBP y posteriormente se hizo el manejo pertinente, con mejoría sintomática del prurito y de las manifestaciones cutáneas secundarias.
