Abstract

The anomalous origin of the coronary arteries is rare, may have different presentations, usually without any pathological disruption; when originating from the contralateral ostium and its course up to the usual localization pass between the aorta and the pulmonary artery may lead to acute coronary syndromes and sudden death. This case refers to a 55-year old patient with acute MI with non-ST segment elevation, undergoing cardiac catheterization that identified atherosclerotic disease and anomalous origin of the left coronary artery in the right ostium. A coronary angioplasty was performed, and the study was complemented with coronary CT angiography that showed a left coronary truncus originating in the right sinus of Valsalva with the left anterior coronary artery running posterior to the aorta. No additional interventions were required.
