Abstract

Introduction: the objective of asthma management is to control the condition and improve the patient’s quality of life (QL). Methodology: cohort study with a before and after design. A comprehensive program, including group and individual education, scheduled and priority pulmonologist appointments, respiratory care personnel evaluation, and a standardized clinical guideline, was applied for 1 year to children between 2 and 15 years of both sexes. Asthma control, QL (Paediatric Asthma Quality of Life Questionnaire and Paediatric Asthma Caregiver´s Quality of Life Questionnaire) and spirometry were evaluated at the beginning and 1 year after. The number of asthmatic crisis, absent days from school, emergency room visits, hospitalizations and LQ variation were measured. The t Student and McNemar tests were used. Results: one hundred sixty eight children were included, 58% male, with a mean age of 7,4 + 3,2, 89% with severe or moderate asthma. The quality of life improved significantly both in children (initial global score: 4,6 ± 1,3 vs. final: 5,6 ± 1,1) and their parents (initial: 3,5 ± 1,5 vs. 5,4 ±1,2) (p<0,001). Compared to last year, the paired analysis showed a significant decrease in the number of emergency room visits (125 vs. 33) and hospitalizations (56 vs. 8) due to asthma exacerbations (p<0,001). Conclusions: a comprehensive program for asthma, which has education as its main component, improves the quality of life of both the patient and their caregivers. It also reduces the number of visits to the emergency room and hospitalizations.
