Abstract

Introduction: short bowel syndrome (SBS) is defined as intestinal failure, which causes malabsorption and malnutrition. The goal of treatment is to achieve intestinal rehabilitation allowing the patient to live without parenteral nutrition. Therefore surgical techniques are studied as an alternative therapy. Objectives: to evaluate the effectiveness of isoperistaltic colonic interposition (ICI) in the surgical management of SBS in animals biomodels. Methods: we performed a controlled pilot study in 20 pigs, an ICI group and a control group, with 10 repetitions in each set. Results: there were significant differences between both groups with better results for the ICI group regarding weight, hemoglobin, hematocrit, lymphocyte count, cholesterol, total protein, albumin, and remaining small intestine and interposed colon diameters. Histological changes were evident in the colon interposed presenting increased mucosal thickness and presence of connivents valves similar to those present in the small intestine, but coated with colonic epithelium. Conclusions: the ICI represents a surgical alternative in the management of SBS, showing better nutritional and weight changes. Additionally there are significant histological changes in the mucosa of colon interposed suggesting intestinal rehabilitation that could promote absorption of nutrients at this level.
