Resumen
Introducción: el avance en el entendimiento de la fibromialgia, como una patología de etiología multifactorial, con componentes biológicos, psicológicos y sociales, en la cual se encuentra con frecuencia comorbilidad psiquiátrica justifica evaluar la eficacia y seguridad del tratamiento especialmente de los antidepresivos. Método: búsqueda en la literatura biomédica en Medline, OVID, Proquest, Scielo y EMBASE, cruzando la expresión MeSH fibromialgya con pathophysiology y clinical trial, delimitando los resultados a los estudios clínicos, revisiones sistemáticas y/o metanálisis en español o inglés en los últimos 25 años en la población adulta. Resultados: se seleccionaron los artículos arrojados por nuestra búsqueda, considerados de relevancia clínica por la calidad metodológica y los resultados en términos de seguridad, eficacia y efectividad. Conclusiones: la FM es una enfermedad generalizada, incapacitante, de etiología multifactorial en donde los factores psicosociales y las comorbilidades psiquiátricas son parte fundamental en el desarrollo y mantenimiento de la misma. La combinación del tratamiento farmacológico y no farmacológico sigue siendo la recomendación más importante en los pacientes con FM. Los antidepresivos son una opción útil y segura en el tratamiento de la FM especialmente los antidepresivos triciclicos y los antidepresivos duales como la duloxetina y el milnacipran.
